Position Objective:
The Health Coach is an operational role and is responsible for helping individuals improve their overall health and well-being. This position will work closely with patients to achieve health care milestones, including appointment scheduling, follow-up after adverse health events, medication adherence, and other healthy behaviors.
Responsibilities:
Communicate and facilitate Transition of Care Management (TCM) telephonically between patients and medical offices as assigned
Contribute to reduced readmission risks through education, care coordination, benefit utilization, and timely documentation
Wellness Visit Appointment scheduling via telephonic outreach
Conduct Social Determinant of Health Survey outreach
Help patients maintain medication adherence by confirming or coordinating prescription fills for certain medication
Maintain accurate and confidential client records
Additional duties as assigned
Position Requirements/ Skills:
Bachelor's degree in a related field (e.g., health promotion, nutrition, psychology)
1 year of experience working as a Health Coach preferred
Strong knowledge of health and wellness principles, behavior change theories, and evidence-based coaching techniques
Proficiency in using technology and software applications relevant to health coaching
Demonstrated commitment to continuous learning and professional development in the field of health coaching
Proficient with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration
Physical Requirements:
Ability to remain in a stationary position, often standing or sitting for prolonged periods of time
Communicating with others to exchange information
Repeating motions that may include the wrist, hands, and/or fingers
Assessing the accuracy, neatness, and thoroughness of work assigned
Must be able to lift at least 15lbs at times
