Responsibilities

Position Objective:

 

The Health Coach is an operational role and is responsible for helping individuals improve their overall health and well-being. This position will work closely with patients to achieve health care milestones, including appointment scheduling, follow-up after adverse health events, medication adherence,  and other healthy behaviors. 

 

Responsibilities:

 

  • Communicate and facilitate Transition of Care Management (TCM) telephonically between patients and medical offices as assigned

  • Contribute to reduced readmission risks through education, care coordination, benefit utilization, and timely documentation

  • Wellness Visit Appointment scheduling via telephonic outreach

  • Conduct Social Determinant of Health Survey outreach

  • Help patients maintain medication adherence by confirming or coordinating prescription fills for certain medication

  • Maintain accurate and confidential client records

  • Additional duties as assigned



Position Requirements/ Skills:

 

  • Bachelor's degree in a related field (e.g., health promotion, nutrition, psychology)

  • 1  year of experience working as a Health Coach preferred

  • Strong knowledge of health and wellness principles, behavior change theories, and evidence-based coaching techniques

  • Proficiency in using technology and software applications relevant to health coaching

  • Demonstrated commitment to continuous learning and professional development in the field of health coaching

  • Proficient with Google Suite (Drive, Docs, Sheets, Slides) and Microsoft Office (Word, Excel, PowerPoint) for real-time collaboration

 

Physical Requirements:

 

  • Ability to remain in a stationary position, often standing or sitting for prolonged periods of time

  • Communicating with others to exchange information

  • Repeating motions that may include the wrist, hands, and/or fingers

  • Assessing the accuracy, neatness, and thoroughness of work assigned

  • Must be able to lift at least 15lbs at times
